Assessment of Patient’s Perceptions Essay

Caring has been widely discussed in the health care professions, especially in nursing which is considered to be one of the caring professions (Boykin and Schoenhofer 2001). Watson (1985) describes caring as a moral ideal of nursing. According to Watson, caring preserves human dignity in cure dominated health care systems and becomes a standard by which cure is measured (Watson 1988c, p. 177). Research literature indicates that the assessment of quality of care from the patient’s perspective has been operationalised as patient satisfaction (Dufrene 2000). Nurse caring has been related to patient satisfaction in western literature (Wolf et al 2003; Wolf et al 1998; Forbes and Brown 1995; Boyle et al 1989). There are so many factors that influence patients’ perceptions of care given by the opposite sex. Some of these are: Age, ethnicity, gender, socio-economic and health status. The most consistent finding has been related to age. Older patients tend to be more satisfied with their health care. Studies that have looked at ethnicity have generally held that being a member of a minority group is associated with lower rates of satisfaction. Studies on the effect of gender show that women tend to be less satisfied and other studies show the opposite. Most studies have found that individuals of lower socio-economic status and less education tend to be less satisfied with their health care. Other studies have shown that poorer satisfaction with care is associated with experiencing worry, depression, fear or hopelessness, as is having a psychiatric diagnosis such as schizophrenia, post-traumatic stress disorder or drug abuse. Health status can greatly influence satisfaction of an individual (Hall, 1990). It is important to examine the relationships of nurse caring to patient satisfaction in Cape Coast because of severe staff shortages, heavy workload, and low salaries in Cape Coast Hospitals. Nursing literature had long recognized importance of these perceptions as major factors militating against prompt and effective service delivery in the care environment. One of the critical roles and ethics of the nurse is to treat all patients irrespective of their gender, colour, creed, political inclination and religious affiliations. High quality nurse-client communication is the backbone of the art and science of nursing. It has a significant impact on patient well-being as well as the quality and outcome of nursing care, and is related to patients’ overall satisfaction with their care. The maintenance of high nurse patient communication also depends on the nurse and patient. The quality of care in a hospital has been shown to be influenced by several factors including: inadequate nursing staff, lack of regular water supply on wards, too much nursing documentation, too long waiting time, and lack of specialized nurses. In Ghana, there is crisis in nurse-client communication evidence from four sources. These are personal observation, anecdotes from client and their families, media reports, and official health reports. Although there is ample evidence to demonstrate that most nurses are females, one cannot discount the enormous services that male nurses provide irrespective of their gender. The ever increasing number of female nurses compared to male nurses in our wards, coupled with patient dissatisfaction toward treatment received has raised a lot of questions as to whether there is a perceived idea that male nurses offer better care or otherwise. With the introduction of Health Insurance a few years ago, more patients report to the hospital and are also consciously aware of their rights and responsibilities as patients and hence will demand for better service delivery from care givers. Recently, patient dissatisfaction has been on the ascendancy in several hospitals across Ghana including University of Cape Coast Hospital. This has raised a lot of concerns about the perceptions patients have regarding the care given by male nurses.

Mental Health

One of the many challenges that human service professionals e.g. social workers, counselors, and other professional helpers encounter in the practice of their profession is the need to understand the cultural, language, social and economic problems of people and their families in order to extend appropriate and effective services to people especially those who belong to the minority groups. Any human services worker is bound to meet countless people with diversified culture. Regardless of whether they are Asians, Hispanics, Europeans, or African-Americans, it is the duty of human service professionals to extend their social services without any regard to the persons’ culture or social status.   Everyone deserves to be treated equally.   Although we may hear stories about the existence of discrimination, we could not attest if such is true or untrue or if social services were denied due to the person’s race, sexual orientation, disability or the color of their skins. If we are in a profession to assist people in coping with unemployment, substance abuse, disability, social problems, child abuse, and spousal abuse among others, our main concern must be in the exercise of our profession and take into consideration our professional ethics and most importantly the welfare of those who are in need of our services.   After all, we are all humans in need of basic services.   We only differ in our beliefs and cultural background. Culture The expansion of our diverse cultural growth is due to the civil rights movement that began in the 1950s, in which African Americans, women, gays and lesbians, people with disabilities and other minority groups alerted the country to their distinct identities and long histories of oppression.[1]   The other is the growing number of new immigrants to this country who bring with them unique cultural, language, religious, and political backgrounds as a result of the internal displacement within their own countries, torture, political oppression, and extreme poverty.[2] These days, people of various races are everywhere.   They could be our next door neighbors, seatmates, friends, co-workers, patients, or chatmates in the internet. Culture is the shared patterns of behaviors and interactions, cognitive constructs, and affective understanding that are learned through a process of socialization which identify the members of a culture group while distinguishing those of another group.[3]   For me, culture is our identity.   It is what we take with us and what sets our community or race apart from other groups.   It may be unique or common, rational or irrational, acceptable or unacceptable but it is something that we have learned, shared and valued. Challenges of Human Service Professionals In other families and in most communities, people speak a language, attend religious or traditional ceremonies, adhere to some beliefs, participate in rituals, strictly observe the intake of foods that are acceptable in their religion and wear costumes that identify their culture.   I grew up with a set of culture handed down by my parents and I had my share of painful experience brought about by my race. When I begin my human services practice and meet people with diversified culture, I know that my personal beliefs and culture may either be similar or different from their own beliefs and cultural practices.   One of the major problems that human service professionals like me would encounter is the language differences as some immigrants could not speak the English language. Although there may be interpreters who are willing to assist us as we conduct interviews and accommodate the needs of our diverse clients, there is nothing more comfortable than being able to speak in a language that both my clients and I can speak and understand well.   Since there are many languages as there are so much non-English speaking immigrants, a lot of human service professionals may find it difficult to learn a foreign knowledge especially when there are piles of cases, reports, and advocacies to work on.   In addition, it takes years of constant practice before one can be fluent enough to speak and understand a native speaker. Although professionals have good intentions in doing what is best for their clients, some African American families look at professionals who provide parent education and support in their homes as taking on roles such as child’s aunt or mother’s sister.[4]   When clients resent the providers’ support, trust is no longer built and they may tend to reject any future support or would no longer seek any professional services. A problem in accepting and understanding the risk factors for the immigrant children and their families is another challenge for the human service professionals.   Sleeping arrangements where children sleep in the same bed or same room as their parents may seem inappropriate in regard to some emotional health and safety issues but typical in terms of custom and nurturance as this risk factor for the majority may be an expression of closeness of kin for the immigrant families. [5] A lot of human service professionals may not be trusted due to the suspicions and fear of the Latinos and some Asian people that they would be deported since a lot of them entered the country illegally, are overstaying in the host country and have not acquired an immigrant status [6]. Personal Discrimination Personally, I feel that discrimination towards another person may be influenced by the negative or painful experience that one has suffered from in the past.   For instance, if an African American child was sexually abused by a white American, she may have a personal hated and would resent having a close relationship with white Americans.   The hated and negative feelings may only be healed when proper intervention is provided.   Hence, without the involvement of any professional help e.g. counseling, discrimination towards a specific race or gender will always remain. In the practice of my profession, I must be fair to any person regardless of their culture, language, gender, religion, political background and economic status.   When I was young, I became a victim of physical abuse by my classmates and schoolmates due to my race.   As a result of this experience, it could be possible that I would be bias towards people who are abusive due to the negative experiences that I had experienced in the past or I would be a good advocate to those who are in a similar predicament.   For me, discrimination cannot be totally eliminated.   It could only be prevented. I believe that my deliberate or unintentional practice of discrimination would be prevented if I will examine my own cultural background, become aware of my personal bias and when I am given the proper intervention and the appropriate training so I could accept any person in his or her totality.   Before I become a certified human service provider, I need to understand people’s differences in the areas of tradition, values, family systems, sexual orientations, and spiritual beliefs, and I must be culturally competent to deliver culturally competent services. In addition to being culturally competent, I must be sensitive and tolerant to cultural diversity and have a wider scope of awareness of how my clients’ experience their uniqueness and deal with their differences and similarities as one of the mission of the human service professionals is to enhance people’s well being, meet their basic needs and to empower those who are victims of oppression, discrimination, injustice and poverty. Conclusion Human service professionals will always encounter new situations and various clients in the practice of their profession.   They also play a great role in providing intervention, economic and social services and linkages in education and training, health, housing and employment.   In order to fully address the diverse needs of individuals and families, the human service workers must be culturally competent and open to diversity. Belonging Related Texts Essay

What do you think the most powerful influences that impact on an individual’s sense of belonging? * Strictly Ballroom by Baz Lurhmann * The Red Tree by Shaun Tan * Who you are by Jessie J You will almost always find where you belong if you search for it. So ultimately a sense of belonging comes down to perception. This starts from places and/ or relationships, which potentially alter your understanding or you and the world around you, so you can accept the person you are and your individual identity by creating this sense of belonging. In strictly ballroom by Baz Lurhmann, The Red Tree by Shaun Tan and who you are by Jessie J the composers use a wide range of techniques to convey the ideas belonging through forcible authority, challenging authority and alienation. These are illustrated through the concept of belonging to a person or place. These three ideas demonstrate what the most powerful influences are that can cause someone to feel a part of something or not. Forcible authority is illustrated in the film ‘Strictly Ballroom’ by Baz Lurhmann. Forcible authority is when a person or group has the power to make you feel a certain way that you may not agree on and make you feel as though you do or don’t belong. This is conveyed in the film when Barry Fife the president of Australian Dance Federation (ADF) forces Scott Hastings to dance a particular way. The forcible authority is demonstrated in Strictly Ballroom with close ups and bright lighting on Barry’s face. This creates attention and power to the audience’s concept on Barry. Scott then feels isolated from the ADF as a result of Barry’s ideas for the ADF. Forcible Authority is also shown in ‘The Red Tree’ by Shaun Tan. This is conveyed through visual techniques of the little girl standing alone in many pages of the book. The concept of a powerful influence is a little red leaf with â€Å"without sense or reason† this demonstrates herself as a symbol of the ‘little red leaf’ with many factors contributing to that idea. E. g. ‘The little girl with the red hair’ The little girl becomes frustrated by society and not being able to find her place or where she feels she belongs without sense or reason. Here the most powerful influences that influence the little girl is her on mind set on other people and how she see’s everyone trying to conform and belong to a place she hasn’t been nor understands. Forcible authority is again conveyed in the song ‘who you are’ by Jessie J through the singers lyrics. Jessie illustrates a strong opinion on society’s sense of belonging when she states â€Å"forget how to fit the mold, yeah! † this informs the audience that society’s conception of belonging is based on a mould and she feels out casted because she doesn’t know how to find her place in society anymore. Jessie feels as though society has clung to a certain way of thinking and living, this is because of the forcible authority, which is the society as a powerful influence on Jessie’s sense of belonging. Challenging Authority is another idea shown in ‘strictly Ballroom’ this can be when you choose to do something about following other rules from a higher authority. This is illustrated when Scott is introduced to Fran’s grandmother Ya Ya. She explains that dancing comes from the heart. The close ups of Ya Ya’s hands beating the traditional rhythm of the Paso Doble on Scott’s chest gives both Scott and Fran the inspiration to dance their own moves which demonstrates to the Audience how they are challenging authority and now have somewhere to belong to. Challenging authority is illustrated in ‘The Red Tree’ when the little girl struggles to find herself in society; this becomes an issue throughout the whole book, always feeling as though she didn’t belong. The very last page is her standing in her room with a large Red Tree filled with lots of red leaves and you can see that she has accepted herself in the society. She has done this because she has a glowing smile on her face. Jessie demonstrates challenging authority in â€Å"Who you are† by giving advice to other people to be yourself instead of living a lie and following society’s rules. This is illustrated when Jessie sings â€Å"Don’t lose who you are in the blur of the stars! †Alienation is the estrangement of somebody who is forced or unforced to distance people from each other or of people from what is important or meaningful to them. Strictly Ballroom demonstrates many moments of alienation, one particularly is Scott feeling as though he doesn’t belong in the ADF because he doesn’t confine with the ballroom dancing rules. This is illustrated when himself and Fran dance their own steps and cause a stir in the ADF judging, this is a powerful impact on belonging because if you don’t follow the rules you are forced to feel neglected. Alienation is also illustrated in The Red Tree through pictures and descriptive language. The visual technique as quoted, â€Å"nobody understands†, It is raining and the brushstrokes are soft but distinctive nd the colours are dull and dark creating the audiences idea of how the girl is feeling. The girl is a seclusion to society showing her being an ‘outsider’, the visual techniques are a powerful influence to belonging and clearly demonstrate how the girl feels alienated by society. Jessie J also shows alienation in ‘Who you are’ b y the visual technique in the film clip, Jessie sings â€Å"Sometimes it’s hard to follow your heart. † In this particular part she is sitting in an empty bathroom with dim lighting, this demonstrates her feelings of loneliness and confusion. Jessie feels confused because she doesn’t know what the right thing to do is and this causes her alienation to the environment she is in, this particular songs provides proof with her excluding herself from society until she makes up her mind. Jessie being the most powerful influence as she is the only one making the decision to alienate herself. Strictly Ballroom by Baz Lurhmann , The Red Tree by Shaun Tan and Who you are by Jessie J all convey powerful influences such as authority to demonstrate people belonging and not belonging . This is conveyed through the techniques of forcible authority, challenging authority and alienation.

Reviewing Recidivism In The Criminal Justice System Criminology Essay Recidivism, in the criminal justice context, is most simply defined as a relapse into criminal behavior (Merriam-Webster Online, 2010). Once a convicted criminal has served his or her time and is released from incarceration, in a perfect world, they would never return. One might assume that having lost all individual liberties, a person would do everything within their power to not ever be in that predicament again. However, against all logic, this is not always the case. When an ex-convict reoffends and is incarcerated again, he or she has then become a recidivism statistic. So the question is, Why does this happen? Does not the thought of untainted freedom appeal to these individuals? Secondly, does society have a role in ensuring these released prisoners have the skills necessary to progress forward as productive members of society; never to return to the prison system? Whose responsibility is it to ensure those who walk beyond the prison wall do not return? And third, with the nu mber of people in prisons and jails (in America) at an all time high, these are questions that beg to be answered. Since the burden of financing prisons falls upon the American tax-payers, it is of benefit to them to invest in efforts geared towards reducing recidivism. All of society has a large stake in keeping offenders out of prison, which in turn decreases the amount of new prisons which need to be built, staffed and funded. With that said, in order that the issue of recidivism be addressed, all involved parties must ultimately agree upon the best course of action. Many in the correctional field erroneously assume that what works to reduce re-offending rates for one individual may work for all. Thus, many programs in place today utilize a standard approach for the masses; recalling one instance in which that particular approach was successful. One side states the best approach to reducing recidivism is more discipline; harsher sentences and longer prison stays. Yet another faction may stand behind the various rehabilitation approaches which include: diversion programs, half-way house stays, treatment programs while incarcerated, work-release programs, substance abuse programs and 12-step programs to name a few. Furthermore, an additional barrier to any approach is the responsibility of correctional facilities to prove what works, via statistics and the like to enable such programs to be funded. So, what really works? Those in the criminal justice community (and scholars in general) make great claims as to what is most effective at reducing recidivism. Every political party, prison administrator, police officer or correctional officer has their own personal analysis on the matter. Nevertheless, as with any concern, before mapping out a solution the problem must first be properly identified. Unfortunately, recidivism is plagued with a number of unique problems which contribute tiny parts to the whole. One great contributor is the fact that many of the prisoners, before even becoming incarcerated, cannot read or write, have very little job skills, have anti-social behavior problems and may even be mentally unstable? With these issues comes the instigator of little to no treatment. Are the correctional institutions able to address these issues or are they simply ignoring them in the spirit of simple incapacitation? Although there is scientific evidence that rehabilitation really does work (even if on ly part of the time), many stake their claims on harsher punishment; retribution. Yes, there has been controversy regarding previous research based on conflicting definitions of recidivism and faulty statistics. Even so, exact definitions or not, the greatest harm to date is the application of a one size fits all approach. In this paper Ill review the various schools of thought and outline programs in use today. Secondly, Ill summarize the programs and approaches which seem to garner the attention of experts, via empirical evidence and scientific research, as possessing the greatest hope for reducing the rates of recidivism; even if only slightly. REHABILITATION Criminal justice policies were not always as punitive as they are today. During the 1950s and 1960s, most Americans held the optimistic view that offenders could be reformed and that it was the responsibility of the criminal justice system to do so (Pager, 2007). This mindset reflects societys tolerance and sympathy at the time. During this period the Federal Bureau of Prisons emphasized rehabilitation as a correctional tool for use in Federal prisons. This Medical Model, a term by which the rehabilitation movement was referred, placed offenders through treatment programs which included counseling, education and vocational training (UNICOR, 2010). Prisoners were taught vocational skills, attended educational courses, completed high-school course work, and in some instances received college degrees. Rehabilitation was a key part of the United States policy on incarceration and prisons in general (Benson, 2003). Another implicit motive for these rehabilitation efforts was the fact that a large majority of the prison population suffers from mental illness. This reality is no secret. Inmates, because of their risky lifestyles, poor access to health care, and substance abuse, are less healthy both physically and mentally (Petersilia, 2003). According to a Bureau of Justice Statistics Report, in 1997 an approximately 347,000 prisoners, both state and Federal, reported having a physical impairment or mental condition (Maruschak Beck, 2001). The severity of these illnesses can range from mild mental issues to severe illness such as schizophrenia. During the Medical Model era, and still today this issue lacks the full attention of correctional institutions and the criminal justice community. Nevertheless, treatment was offered for mental health issues during this time. Up until the mid-1970s offenders were often given mandatory therapy and drug treatment as part of their prison sentence. Many had substance abuse problems and were treated for such. Convicted criminals were seen as needing supervision, re-socialization, and support in acquiring the coping skills, behavioral therapy, and vocational skills which form the foundation for reentering society. At that time it was generally believed that counseling, education, and job training were central to criminal desistance and that active intervention could have lasting effects (Pager, 2007). As previously mentioned, many offenders enter prison as illiterates with very little marketable job skills, drug problems and mentally health issues. Thus, this approach seemed practical. Although necessary this rehabilitative approach would not last long enough. Unfortunately, when the get tough on crime initiatives rolled in the rehabilitation portion of punishment was rolled out. Former President Ronald Reagan took it upon himself to focus the nations attention toward the problems of drug use and distribution. These get tough on crime initiatives, a product of Reagans war on drugs, caused prison populations to explode almost instantaneously. Between 1980 and 1990, the annual number of drug offenders admitted to state prisons increased tenfold (Pager, 2007). With the inundation to the prison population it was technically impossible to effectively rehabilitate each and every prisoner. This was especially true with the increase in prisoner sentences being tied more specifically to drug offenses, which meant substance abuse treatment for all. Along with these new initiatives came a more corrective approach towards punishment which left little room for rehabilitation. Along with the prison explosion of the 1980s and 1990s came a decrease in financial backing for the departments of parole (Pager, 2007). This decrease, purportedly tied to the war on drugs, put a strain on parole officers role in the rehabilitation and reintegration process. Tasked with reintegrating offenders back into society, the departments of parole as a whole are an integral part of the reentry movement and effort to reduce recidivism. An offender is generally considered to be rehabilitated when he or she no longer represents a threat to members of the community and are therefore believed to be fit to live in that community (Gaines Miller, 2007). Parole officers are (or were) responsible for the post release portion of this rehabilitation effort; getting the ex-con back in touch with his or her community. After the prison explosion parole officers were now finding themselves with almost three times more the amount of caseloads then previously charged with. This reduction in ti me and increase in case loads meant less assistance and more correction and supervision. Yet another rehabilitative tool was being left behind. A great majority of the American public, ex-convicts included, have always had unfavorable opinions of the department of parole and probation officers. Now trust in the department was dwindling even further. WHAT WORKS Two (2) of the largest studies on recidivism were conducted by the Bureau of Justice Statistics in 1983 and again in 1994 (the fourth and fifth BJS study of offenders released from prison and their re-involvement in crime). The most obvious lesson learned from these studies was that from 1984 to the 1994 there was a 5% increase in recidivism for those prisoners released (Langan Levin, 2002). Thus, it would seem that the practices currently being utilized to address the concern of recidivism are, or have been, somewhat ineffective. Those released in 1994 were not only re-arrested in greater numbers than those released in 1983. But they were also re-arrested sooner after release and for graver offenses (Beck Shipley, 1989). The greatest chance for reoffending took place within the first year of release from prison (Langan Levin, 2002). As mentioned previously and as pointed out by both the BJS studies on recidivism, there are several contributing factors which come into play when co nsidering whether or not an individual will reoffend. Factor such as education level, prior arrests and/or previous incarcerations, age, gender, type of offense, amount of time served, and ethnicity. These characteristics, to name a few, are part of a reoffended equation. These characteristics can also be utilized to form a risk assessment, which may identify those who are at the greatest risk of reoffending. This insight again solidifies the notion that not each person can be addressed utilizing the same approach. Alongside research conducted by the Bureau of Justice Statistics stands a large body of study collectively known as the what works literature (Latessa Lowenkamp, What Works in Reducing Recidivism, 2006). The what works literature has, with the assistance of evidence-based research, further confirmed that what is good for the geese is not always good for the gander. As continuously stated, not all approaches work for every offender. In addition, even the most successful approaches are not successful every time (Hooley, 2010). Its unfortunate that some offenders simply cannot function outside of prison walls. There are those criminals who will ultimately remain criminals stuck in a cycle of reoffending. Part of the what works literature addresses this issue by addressing the risk and needs of each individual inmate or ex-convict. Dr. Edward J. Latessa and Dr. Christopher Lowenkamp, both Professors at the University of Cincinnati have dedicated much of their research to the study of recidivism. Drs. Latessa and Lowenkamp (2005) have a recurring theme that the components of effective correctional intervention, including offender reentry programs, have already been demonstrated and confirmed time and time again (p.72). Many academics (Petersilia 2003, Pager 2007, Hooley 2010) profess the fact that there are programs that have been scientifically proven more successful than others. According to Latessa and Lowenkamp (2005) the setback in recidivism stems from correctional institutions disregarding this large body of evidence in favor of quick fixes or the latest trends. According to Hooley (2010) and several authors of what works literature in addition to science based strategies and writings put out by the National Institute of Corrections, in order for a correctional system to be effective at reducing recidivism it must adhere to several core concepts. The most important core concept is a risk assessment; who to target (Latessa Lowenkamp, What Works in Reducing Recidivism, 2006). Included in this risk assessment are several characteristics identified in the Special Reports conducted by the BJS as well as others. These characteristics include, but are not limited to: age (the younger the prisoner when released, the higher the rate of recidivism), attitude (does the offender display anti-social attributes), criminal history (will he or she continue to hang around with other who cause trouble), motivation (will the offender seek and keep employment), and attitude (does the offender accept responsibility for their actions either good or bad). Once th ose at risk are identified, resources should only be expended on offenders with the highest probability of reoffending (Hooley, 2010). In the age of do more with less its imperative, both for taxpayers and the correctional system, to utilize funds and resources where they will most likely to have the biggest impact. It has also been proven that it is possible to do more harm than good if low risk offenders are paired and treated with high risk offenders. In Latessa and Lowenkamps 2005 article in the Ohio Judicial Conference, they speak of a second principle to be addressed; criminogenic needs. The most effective programs assess, address and meet the individuals criminogenic needs. These needs include who they hang out with, their attitudes, beliefs and values, their lack of problem solving, substance abuse issues, and employment status to name a few (Latessa Lowenkamp, What are Criminogenic Needs and Why are they important, 2005). They have the largest influence on what motivates the individual to reoffend. One such example is a recently released inmate who continues to associate with anti-social and criminal friends. Chances are they will be more likely to reoffend. Programs such as boot camps and other programs which address non-criminogenic needs are by themselves unsuccessful as they tend to not focus on crime-producing needs (Latessa Lowenkamp, What Works in Reducing Recidivism, 2006). Although self-esteem and discipline build ing programs can be helpful for a short period of time, those that fail to address the attributes which encourage criminal behavior will never be successful in the long term. While static criminogenic needs such as a prior criminal record and family criminality cant be change, dynamic criminogenic needs such as those listed above can be influenced to reduce the individuals chance of reentering the criminal justice system (Latessa Lowenkamp, What are Criminogenic Needs and Why are they important, 2005). The next concept is referred to as the treatment principle. This principle addresses the how to go about targeting offenders needs. Evidence-based programs that are not only behavioral in nature, but are also delivered by a well-trained staff have been proven to be successful (Hooley, 2010). Some offenders may have to be taught a whole new way of thinking. Cognitive behavioral therapy that focuses on current factors influencing behavior that are action oriented with role-playing involved have a tremendous influence on improving anti-social behavior. A four to one ratio of positive reinforcement over sanctions has been found to be one of the most effective forms of treatment (Hooley, 2010). Many inmates have had no positive influence in their life or have never received positive reinforcement paired with non-criminal activity. Thus, in the treatment principal social learning is reinforced, via psychotherapy that emphasizes the important role of thinking in how we feel and what we do ( National Association of Cognitive Behavioral Therapists, 1996-2010). This encourages the individual to become less anti-social and motivated to do better. But above all, none of the above mentioned strategies are useful unless they can be delivered by a well-trained and dedicated staff. This applies to programs delivered both inside and outside of prison walls. In What Works in Reducing Recidivism, Latessa and Lowenkamp (2006) name this as the fifth core concept. All parties involved in the process of rehabilitating the offenders attitudes and behaviors must be on-board and on the same page. Those implicated must be trained, competent, consistent and sensitive to the offenders needs. This support is imperative. This requires on-going support as staying away from negative influences in the form of old friends and family may prove easier than staying away from drugs (Hooley, 2010). Dr. Lateesa cites recent studies which have demonstrated that even evidence-based programs can increase recidivism if not competently delivered (as cited by Washington Institute for Public Policy, 2004). THE REENTRY MOVEMENT There is a vast amount of anecdotal and empirical evidence pointing out the rise in prison populations. Furthermore, an abundance of reports signal an increased number of ex-convicts being released into the community. Hence, it only makes sense to focus more attention on pre-release programs and training. Much of the incentive surrounding the reentry movement have sprung from pressure on public policy makers to provide treatments and services that will smooth the integration process while reducing recidivism (Nunez-Neto, 2008). Approximately 650,000 people are released from prisons every year to return to communities that may or not welcome their return (U.S. Dept of Justice, 2010). With such a large number of annual releases, the most important question to be asked is whether or not those released are equipped to be productive members of society. Without the proper attention and equipment (both physically and mentally), they will not be prepared. It is with this information that a reentry movement of sorts has surfaced and gained great momentum during the past century. Correctional facilities are striving to make the transitioned from the punishment mindset to one of rehabilitation and preparation for prisoner reentry and reintegration. In the words of Joan Petersilia of the University of California at Irvine, prisoner reentry encompasses all activities and programming conducted to prepare ex-convicts to return safely to the community and to live as law abiding citizens (Petersilia, 2003). The correctional community at large is slowly coming to the realization that, without the tools required to function in what may be a whole new world, a recently released convict may have but one option; to resort to the criminal tendencies which proved useful prior to his or her incarceration. Part of the reentry progress has come with backing from the Federal government. On April 9, 2008 the Second Chance Act of 2007 was enacted. H.R. 1593 outlines the purposes of The Second Chance Act of 2007 as the following: To break the cycle of criminal recidivism, increase public safety, and help better address the growing population of criminal offenders who return to their communities and commit new crimes. To rebuild ties between offenders and their families and to encourage the development and support of programs that reduce recidivism. For example: substance abuse treatment, alternatives to incarceration, and comprehensive reentry services. To protect society and promote law-abiding conduct by providing necessary services to offenders. To assist offenders reentering the community with transitional services for a sufficient amount of time after incarceration. To provide offenders in prisons and jails with educational, literacy, vocational, and job placement services to facilitate re-entry into the community. (The Second Chance Act, 2008) This original Second Chance Act of 2007 was a way for Congress to authorize a number of new programs and expanded the current offender reentry grant program at the Department of Justice (Nunez-Neto, 2008). It also assists the Federal Bureau of Prisons with its offender reentry programs in modifying components which prove most successful for federal ex-offenders. And it establishes a prisoner reentry program at the Bureau of Prisons and bestows the agency with new authorities (Nunez-Neto, 2008). This Act, and other similar programs at the state and local levels, is instrumental in giving ex-offenders the boost they need to be more victorious in their quest to make it on the outside. REINTEGRATION Three phases are often associated with offender reentry programs. First there are those programs that take place during incarceration, which aim to prepare convicts for their release. Secondly there are those programs that take place during the convicts release period, which seeks to connect them with the various services they might require once released. And lastly, during the final phase, there are the long-term programs that support the offenders permanent reintegrate into their community and society. This final phase attempts to provide the support and supervision ex-cons require most (Nunez-Neto, 2008). It is also the most crucial phase as what happens during the months following an ex-cons release often determines if and how quickly they will or will not re-offend. As stated previously, from the late 1950s up until the mid-1970s offenders were often given mandatory therapy, drug treatment, behavioral therapy and coping skills during their incarceration period. Rehabilitation efforts were at the forefront of correctional reform. The pattern during this era emphasized rehabilitation and reintegration. Since the early 1980s, views towards these forms of traditional treatment efforts have begun to deteriorate (Kadela Seiter, 2003). With changes in ruling political parties and differing prison administrations also comes change in corrective tactics. Fortunately, the pendulum is returning towards rehabilitation and for the past century rehabilitation has been viewed as not only the most humane goal of punishment, but also as a cost-effective (and possibly crime-reducing) alternative to punishment (Gaines Miller, 2007). Nevertheless, in order to alter the in prison experience prison officials have to accept that prisoner reintegration is part of their core responsibilities (Petersilia, 2003). Correctional staff and prison administration must make extensive efforts to ensure the prisoners are prepared for reentry. This includes not only a return to providing education and treatment, but also promoting and environment that encourages positive relationships, improving life skills and conflict management as opposed to violence and domination (Petersilia, 2003). While incarcerated each day of an inmates life is planned and structured. The inmate is required to make very few decisions on his or her own behalf. Allowing an inmate opportunity to make decisions will reduce the trauma of having to deal with lifes decisions on the outside. Studies show that the most successful programs in the reentry movement are those which begin during the actual incarceration period. During the months, and possibly years, leading to release is when programs which attempt to connect inmates with services theyll require upon release are most important. Again, correctional staff truly engaged in the process must foster collaborations and develop partnerships with the community. These services can include employers willing to participate in work-release programs, family members who participate with the inmate in counseling services, and community services geared toward assistance with housing and transportation to name a few. The list of programs and assistance should be modified to meet the individual needs of each inmate. In the words of Joan Petersilia (2003), Just as welfare reform forced welfare agencies to shift from a dependency model to a model of transition to independence, so too a reentry perspective would force correctional agencies to take practical steps to move prisoners toward independence (p. 174). Even upon release from prison, an ex-convict is labeled by society. What seems to be a great instigator to the crisis of recidivism is the fact that upon being released from prison many a ex-con feel they are still doing time (Pager, 2007). Felons face numerous hurdles in todays world. Many (especially those with drug convictions) are barred from public assistance, which may include housing, food stamps, or, in the case of women, WIC (Women, Infant and Child assistance). Convicted felons cannot vote and in some states they are further disqualified from obtaining a drivers license (Samuels Mukamal, 2004). This is more grounds with which to facilitate the rehabilitation and return of the parole system. Without the Department of Paroles (due to abolishment in some states and determinate-sentencing releases in others) many states have form their own form of mandatory supervision. This supervision is still less individualized and based on risk rather than need, setting supervision levels based primarily on offenders history of criminal behavior (Kadela Seiter, 2003). Thus, even if an inmate has participated in rehabilitation programs while incarcerated and was assisted with reentry by both the prison personnel in conjunction with a well-trained staff, that inmate must still be able to successfully navigate the outside world. EMPLOYMENT Being able to find a job is one of the easiest ways for an ex-convict to get back on his or her feet. With employment comes the ability to pay rent, purchase food and clothing, a boost in self-esteem and, most importantly, the choice of not having to resort to illegal means. In addition, the ex-convict may be returning to a family for whom he or she must now provide. A job for an ex-con in this situation means an already stressful situation becomes less traumatic. For both male and female ex-cons, being able to find a job after being released from prison can only be a boost to their confidence and sense of worth. Research has empirically established a positive link between job stability and reduced criminal offending (Petersilia, 2003). However, for most the odds are against them. To add insult to injury, nearly 80 percent of parole boards nationwide list some form of gainful employment as a standard condition of parole (Pager, 2007). Back in the 1960s and 1970s Wisconsin, New York and Hawaii passed laws forbidding discriminating against ex-convicts based on their criminal record (Pager, 2007). Discrimination of this sort could only be done so if the nature of the crime was directly related to the specific responsibilities of the job (Pager, 2007). Of great note is the fact that only three (3) states have such laws preventing employment discrimination against ex-convicts. The view of the remaining states is indicative of societys judgment as a whole. The majority scrutinizes ex-convicts as untrustworthy, uneducated and unable to be employed for anything but manual labor. Many equate all felons to murderers and rapists. Unless more states pass similar laws, which hold the status of a protected category, similar to protections provided for minorities under Title VII of the Civil Rights Acts, the most important factor in successful reintegration will also be the hardest; finding a job. When applying for any job some of the questions an applicant must answer are those regarding previous criminal history. And in this age of technology many employers also have the option of conducting background check. Combinations of these two facts do not bode well for those with a criminal record. On a positive note, Petersilia (2003) reported that a recent survey revealed employers are becoming more receptive to the idea of hiring an ex-convict if a liaison-a service provider, counseling program or community center-is available to mentor and to help avert any problems (as cited by Wirthlin Worldwide, 2000)(p. 196). This is yet another testimony to the importance of an ex-con having an advocate on the outside. Whether it be a parole officer, a community-based program that has chosen to champion the felons fight, or family members intent on providing for their loved one; assistance plays a pivotal role in the success or failure of the ex-cons struggle. PAROLE The same get tough on crime policies that nearly abolished the rehabilitation mind set toward incarcerated prisoners were also responsible for inadvertently (or purposely depending on who is asked) diminishing the functions of the parole board. This encouraged cooperation amongst prisoners who were highly aware that bad behavior could affect their chance of gaining parole. After the Sentencing Reform Act of 1984, the length of time served by federal convicts for their crimes rose significantly. Sentencing reform statutes and truth-in-sentencing laws had the same effect at the state level (Gaines Miller, 2007). These legislative enactments were attempts to ensure convicts served the majority of their actual sentence; determinate sentencing. Secondly, change came on the heels of crimes committed by inmates who had gained parole and reflected societys belief that those being incarcerated were more violent that before. The down side of determinate sentencing, where most prisoners serve approximately 85% of their sentence, was the abolishment of parole by the federal government and between 14-16 states (Gaines Miller, 2007). In When Prisoners Come Home, Joan Petersilia (2003) presents twelve recommendations on how to reform parole and reentry practices (p. 171). Petersilia (2003) presents reinstituting discretionary release as an incentive for inmates to try to rehabilitate themselves while incarcerated (p. 188). Although inmates are to serve the majority of their sentence, these terms are generally shorter under determinate sentencing. Therefore, they have no motivation to earn good time or show effort toward rehabilitations since their sentences are shorter. And without discretionary parole the board no longer has the power or ability to deny release when it deems necessary. A case in point is Richard Allen Davis whose account is described by Petersilia (2003). Under indeterminate sentencing Davis was denied parole six times. With the Sentencing Reform Act, the state of California (in which Davis was imprisoned) chose to abolish discretionary parole. Thus, in 1993 Davis was mandatorily released due to time served. The changing face of UK Fashion retailing market The clothing retail industry has been and is undergoing significant changes resulting from processes of globalisation, changes in consumer demand as well as changing corporate activities in terms of strategic marketing decisions. The UK clothing retail industry has experienced significant growth which has focused attention on studies in the field with this sector being considered as the one of the most competitive markets in Europe. This is primarily as a result of the concentrated power of larger retailers such as Marks Spencer. One prime issue as a result of globalisation is that there are more and more retailers sourcing globally aiming to provide competitive prices and quality products in the market. Marks Spencer as such then is challenged by this trend particularly from rival discount and supermarket competitors. Research Method The basis of the primary research conducted in this brief was delivering a questionnaire based survey delivered at four retail outlets two of which were Asda and two of which were Marks and Spencer. Opportunistic sampling was used in that customers were approached in the store as they entered and left the premises and asked would they participate. A target of 100 questionnaires to be completed at each store was decided on to achieve a healthy sample size yielding useful results for the analysis. The questionnaire had four sections. The first of these sought general demographic information such as age and sex, the second section sought information related to the frequency of shopping at the stores as well as frequency of shopping elsewhere. The third section sought altitudinal information related to reasons for purchasing in the respective stores while the fourth section and the fourth section sought information on preferences in terms of shopping specifically related to the variables of quality, price and the retail environment. Data was entered into SPSS and analysed according to the objectives of the study. Permission to conduct the questionnaires were sought be local managers in the first instance though for two of the stores further permission had to be sought be regional managers. All respondents were advised of the anonymity of participating in the research and the results of the research were also offered to the managers involved in order to increase the chances of successfully gaining permission to conduct the research. Surveying was conducted during a weekday and at a weekend time in order to enhance the representativeness of the sample in terms of people working and varying their shopping times. Comparison of marketing mix in George at Asda and Marks Spencer Overall 412 questionnaires were completed but 12 of these were discarded due to incomplete or incorrect completed parts. Of the 400 68% were female and 32% male. The age breakdown of the sample showed that some 38% of the total was between the ages of 26-32 or over although in the case of Marks and Spencer shopper the proportion of shoppers in this age group was higher being 49%. Also Asda had a higher proportion of shoppers between the ages of 18-21 than Marks and Spencer the figures being 19% and 9% respectively. Over 90% of the total sample had shopped more than once in each of the stores within the previous month with 45% shopping at least twice a week in either Asda or Marks and Spencer. Of interest was the result that 40% of Marks and Spencer customers indicated that they would shop there regularly (at least once a week) while the figure for Asda was 29%. This may be an indicator of stronger brand loyalty for Marks and Spencer in some respects but given further results examined below may be under threat given attitudinal results related to price and quality. Product The UK clothing retail industry has grown in recent years and reached a value of $38.7 billion in 2004 which represented an increase of 3.7% over the previous year. Womens wear was one of the fastest growing areas in the sector (Datamonitor, 2005). However it is claimed that the growth in revenues within this industrial sector will slow down mainly due to the impact of discount and value adding retailers offering clothing goods at lower prices forcing prices down overall within the industry. Clear indications of this trend can be seen in this case study in that George at Asda and its range achieved significant growth and success compared to overall declining profitability within Marks and Spencer. Since the introduction of clothing ranges by the supermarket in 1990 its product categories have expanded to include womens wear, mens wear and infants-wear. Marks Spencer has operated in the marketplace for decades and offers similar product ranges. While 49% respondents indicated a preference for shopping in Asda for womens clothes 51.2% of customers chose Marks Spencers to purchase womens products. It is obvious that there is slight difference in the choice of product range yet the results also suggest that there a significant percentage (60%) of customers chose George at Asda primarily to buy childrens clothes and in particular to buy items such as school uniforms. One of the main reasons for this arguably can be explained by the lower price yet reasonable quality design of the Georges range. Especially for younger children where school uniforms would be discarded in a relatively short time due to the children growing the price of products offered by Asda can be said to be a significant attractive featuring influencing purchasing decisions. It has been reported that the childrens wear sector in the UK grew by 3% and reached a value of $4.7 billion in 2004. This phenomenon is perhaps attributable to the success of discount stores and supermarket operators who are able to provide cheaper childrens wear at reasonable quality levels (Datamonitor, 2005). From a macro perspective however the decline of birth rates in Britain has resulted in the expectation of a future decline in childrens wear sales. Hence arguably over the longer term it may be the case that with possible future declining sales that this sector will become even more competitive and quite feasibly the number of operators will be reduced in retailing childrens wear. Evidence from this case study suggested that childrens wear product ranges represented 27% of customers purchases in Asdas compared to 10% in MS. Additionally supermarket as generally a daily shopping preference for food provided a relatively convenient shopping environment for women and in particular working women to purchase childrens clothes. This was indicated by some 59% of respondents at the Asda stores stating they had or were intending to purchase items other than clothes as well. It is reasonable to say that as a result of changing roles for women in both social and economic contexts there is increasing pressure such as reduced times available for shopping for women as discussed in literature review. These new consumer behavioural characteristics have resulted in changing retailing forms in the clothing industry to which new entrants such as supermarkets like Asda have capitalised on and responded to quickly as they emerge. In addition more men have become involved in clothing buying behaviour illustrated by the mens wear sector growing 5% in 2004 (Datamonitor, 2005). It was interesting to examine the results of this case study in the comparison of consumer preferences towards mens wear at Asda and MS. In this research 46% chose MS when purchasing mens wear as opposed to 33% in Asda. It is useful to look at this trend in understanding the changing features of the UK clothing retail market bearing in mind major impacts on consumer behaviour within the sector. Firstly Mark Spencer have exploited a branding strategy which established a reputation for high quality goods for a considerable period of time. As a result of price wars between supermarkets and value adding discount stores MS has aimed to focus on quality in order to differentiate it from rivals and position the store further in terms of exclusive designs as well as quality products. However its advertising campaign with the tagline ‘exclusively for everyone’ which was launched five years ago has been demonstrated to have mostly failed with customers turning away from the retailer because of its high prices to lower cost offerings in other retailers (Gardner, 2004). The high figure though mentioned above for MS respondents relating to whether they would shop in the future in the stores perhaps indicates that MS still retains a sizable customer base from which it could build on. However the validity of this assumption can be challenged given the size of the sample of this research and would need further investigation on a larger sample to confirm or deny. As such while MS seems to have failed in maintaining competitive advantage through emphasising quality its supermarket rival Asda has also introduced designer ranges and positioned itself in terms of good design as well as lower price offerings. It is fair to argue then perhaps that MS will not improve its performance by challenging its supermarket rival on pricing and volume. Hence one response has seen MS switching to a branding strategy. Secondly as outlined in the literature review purchasing clothing products should not be seen as indicative of normal commodity buying behaviour because it is to a large extent it is linked with extrinsic factors such as culture and social status which in turn makes the value of brand names an essential element of clothing goods (Entwistle, 2000). Davies (1998) claims that the marketing of own brands is a feature of modern multiple retailing which can also be seen in this case study. Retailers such as Marks Spencer have created brand images by advertising their own products and in the meantime transferred this brand equity into the physical products in their stores. The costs of branding to the retailer are lower than that of marketing other manufacturers brands. This contributes to an understanding and explanation of attitudes towards mens wear purchasing behaviour which in this case saw the brand name of Marks Spencer as a quality clothing producer helping the retailer maintain a critical market share in the mens wear sector. It perhaps also demonstrates that men are less sensitive about price in purchasing clothing goods than female consumers in that brand name and perceptions of quality influence their decision making processes. Jobber (2001) argues that price is often used as an important indicator by customers during the decision making process hence it is useful for fashion retailers who exploit pricing tools to create impacts on customers at the perception and evaluation stages of the purchasing process. As Britains biggest clothing retailer Marks Spencer has established its brand image traditionally on the basis of providing quality clothing goods reflected in higher pricing justified as being good value for money given the quality of the product. In contrast competition at the discount and value adding clothing segment of the market has generated significant threats to MS not only from discount retailers such as Primark but more importantly from new supermarket operators. In this case study George at Asda was chosen in order to provide a useful point of view in understanding the changing environmental contexts of clothing retailing. With a turnover of  £8301.5 Marks Spencers is one of the major clothing retailers occupying a commanding position in the middle market level (FAME, 2005). It is reported though that Marks Spencer has continuously lost market share due to intense competition in the UK clothing retail industry as well as from threats posed by new entrants such as George at Asda which has proven to be a highly successful entrant with the company reporting revenues of $26 billion by the end of January 2005 a significant increase of 19.8% compared to 2004 (Datamonitor, 2005). In contrast to George at Asda, Marks Spencer saw a decrease of 2.6% in revenues with operating profits falling to $1.3 billion in 2005 (Datamonitor, 2005). According to this research customers switched from Marks Spencer to cheaper retailers where quality levels were perceived as being relatively high with George at Asda being for 61% their first choice. The extremely low price of designer products helped Asda penetrate the clo thing retail sector successfully and effectively. It is fair to say that international sourcing and effective supply chain management contributed to a significant degree to the competitive prices in its clothing sector in the same manner as its grocery section. It is also interesting to note that although more than 40% of customers believed that Marks Spencer maintained a better quality level for clothing product ranges based on its reputation and effective quality control programs only 31% respondents considered clothing products at MS as good value for money. As such then the price war between supermarkets and discount stores have driven down the market price for clothing products while improvement of quality in both retailing sectors has become essential in the overall value adding process. As a result as one of the main middle market players Marks Spencer has been threatened both at price and quality levels. A strong financial background enabled Asda to achieve economies of scale and benefits from its integrated supply chain. The power of Asda as a retailing giant helped it negotiate with global suppliers for lower costs which as a result lowered final prices for consumers. Particularly in recent strategies by employing designers the creation of brand name and image of George at Asda contributed to its competitive capabilities. This point of view corresponded to earlier research discussed on consumer behaviour towards clothing buying which suggested that customers are particularly sensitive to price regardless of brand names yet in considering any extra value perceived in a brand name with the purchase customers became less sensitive to this element. This is able to help explain the strategic decision for Asda in building the brand name George at the initial stages of entering this very competitive market. Reflecting this it has been reported that MS had lost its place as the UK’s biggest clothes retailer to its supermarket rival Asda. with MS’s volume share of the clothing and footwear market remaining static at 9.1% while Asda’s George label increased its share from 7.3% to 9.4% in 2004 (Barnes, 2004). As a result the supermarket has grown to be Britain’s biggest clothing retailer after figures produced by Taylor Nelson Sofres which pointed to the volume of the George range at Asda rising to 9.4% compared with MS’s share which was static at 9.1%. However in terms of sales by value MS is still the market leader beating the Wal Mart-owned supermarket by  £3 billion pound (Gardner, 2004). It is thus expected this represents a significant change in the retailing market particularly in consideration of the middle to value adding discount clothing market. This corresponds closely to arguments reviewed in the literature section which suggests that further casualties in the Mid-market and Value Clothing sector are expected driven by increasing competitive levels (Collins, 2005). Supply Chain Management: franchising, merchandising and distribution Since the mid 1990s there has been an evolution in the UK clothing retail industry which has seen increases in both offshore sourcing and international expansion (Jones, 2002). In terms of local apparel manufacturers sales are focused on a few large buyers and Lustgarten (1975) has found that concentration ratios of major buyers have a significant impact on profit margins in the industry. In the case of Marks Spencer its strong position in the supply chain gave them strong negotiation abilities in the sourcing process but the trends of offshore production and overseas sourcing by other retailers has created crucial threats and challenges. The fast fashion strategy introduced by George at Asda in 1990 depends on effective supply chain management especially in the management of sourcing abroad to reduce production costs. Additionally the Wal-Mart owned supermarket enjoys economies of scale from its large number of suppliers globally and as a result good design and fast fashion goods c ould be transported to supermarkets rapidly as with its grocery products. However for Marks Spencer as one of the UK’s leading retailers the franchising group acts in a revolutionary capacity in seeking to respond to these trends effectively (Whitehead, 1991). Marks Spencer has attempted to reduce stock hold by improving the efficiency of its logistics management in terms of lead times through promotional sales for stock goods (Benady, 2005). However the results here were disappointing particularly in relation to womens wear. Market share fell by 5% and UK retail sales fell by 1.7% (Benady, 2005). The problematic aspect of this can be found in the supply chain where too complex a design in the production process and inefficient management strategies resulted in the decrease. Results in this research shows that respondents believed merchandise in the basic sales floor in MS was characterised by a lack of a clear marketing image and that the product ranges were too complex. The retailer has sought to cut down on its sub-brands and volume of stock held in order to introduce fashionable products at competitive prices in a shorter time. However it has been a point of concern that the management of its supply chain relied on management consulta nts rather than sales figures and marketing research on customer preferences. Consequently while UK womens wear sales grew by 4.9% Marks Spencer suffered decreases in terms of market share and operating revenues leading to reduced profitability. Due to the changing nature of the UK clothing retail industry there are revolutionary changes occurring in the supply chain sector. The most important element here can be seen as the increasing amount of sourcing activities from cheaper overseas suppliers such as Eastern European countries as well as far Eastern regions. Additionally bearing in mind the key features of clothing products the principle of shortened response is an integral element of the sector. This is to say lead times in terms of design, production, transportation as well as merchandising have become shorter. In turn benefiting from significant declines in stock holding numbers as well as risks linked with mark down sales clothing retailers are able to provide new ranges products within several weeks with maintained profit margins. As Porter (1980) suggests industrial performance is largely influenced by levels of competition and this is the case for the UK clothing retail sector. The speed element linked with purchasing has also decreased dramatically. This trapped energy causes the Earth’s temperature to rise. Without this process, the global temperature would be a frigid 10 degrees below zero (Salmon). Some scientists believe that human industrialization is increasing the amount of carbon dioxide in the atmosphere causing the global temperature to rise. The resulting theory is global warming.
